DonationsTo request a donation, please visit your neighborhood friends at Polly's Pies and ask for a "Donation Request Form." Completely fill out the "Donation Request Form" and return it to a Coach/Manager from the Polly's Pies Restaurant location you want to receive the donation from. The manager will date and sign your request. The date on the form should be at least three weeks before your event.
